Join our Social Prescribing team and help local residents build confidence, improve wellbeing and take positive steps towards employment.
We're recruiting a Community Link Worker to join our Social Prescribing team and support an innovative project helping local residents overcome barriers to employment.
You will work with people living with musculoskeletal conditions and/or mental health needs who would like support to move towards employment, training or volunteering.
As a Community Link Worker, you will build trusted relationships with residents, helping them identify their goals, improve their wellbeing and access the services, opportunities and practical support that can help them move forward.
You will work closely with health professionals, employment services, community organisations and voluntary-sector partners to provide personalised support. The role will also involve supporting the South London Partnership with digital and marketing materials, developing local networks, and building relationships with partners across South London.
This is an opportunity to join a growing service that has supported people across Merton to improve their health, wellbeing and quality of life for almost a decade. You will be part of a supportive team and help shape an approach that puts people and communities at the centre.
Salary: £31,110 per year
Contract: Full-time, fixed-term for six months; extension subject to funding. Job-share applications will be considered.
Location: Merton, with a mix of community-based work, GP practices, outreach venues, cross-borough networking, Vestry Hall office working and opportunities for home working.
